Flagler County has a total of over 100,000 residents who are insured or uninsured. Limited providers are serving both primary care for adults and pediatrics. However, to meet the needs of our county after hours support may be provided after 6 pm to support the working-class population. The team is committed to helping to reduce some of the challenges of our community through expansion services and a creative a la carte treatment plan. Phoenix Primary Care LLC will also provide medical services after hours via telemedicine or through direct care to meet the acute needs of those who are in need of health care.

SLIDING FEE DISCOUNT

Phoenix Group serves all patients regardless of their insurance or  financial status.  Phoenix Group can provide services through a sliding fee schedule to patients who qualify based on household size and gross income.

Phoenix Group staff is available to assist patients in determining if they are eligible for our Sliding Fee Options. Patients MUST show most recent proof of income for all family members/individuals living in your household; examples of accepted documents include:

  • Previous year’s 1040 tax form
  • One month of pay stubs
  • 1 unemployment stub
  • Food Stamp Award Letter
  • Copy of Social Security or Disability Check
  • Letter from an employer that states your salary or wages.
  • Discount levels for patients who qualify
